چکیده

A 76-year-old woman presented with life-limiting claudication in the left leg. Ankle brachial index (ABI) was 1.07/0.63 (right/left). Duplex ultrasound showed a decreased flow pattern of the left popliteal artery with normal flow pattern in the common femoral artery (CFA), and stenosis or occlusion of the left SFA was strongly suspected. Angiography 1 month later showed severe stenosis in the olymer-free, paclitaxel-coated nitinol drug-eluting stent (Zilver PTX; Cook Medical, Bloomington, IN, USA) is now available for the treatment of peripheral artery disease in the superficial femoral artery (SFA), and durable clinical outcomes compared to the bare-metal stent counterpart have been reported.1–4 Arterial repair after Zilver PTX implantation in the SFA, however, remains uncharacterized. Angioscopic assessment of early phase arterial repair after paclitaxel-coated nitinol drug-eluting stent implantation in the superficial femoral artery.

BACKGROUND Although durable clinical outcomes have been reported, arterial repair after paclitaxel-coated nitinol drug-eluting stent (Zilver PTX) implantation in the superficial femoral artery (SFA) remains unclear. METHODS AND RESULTS Angioscopic evaluation was performed in SFA intra-stent surfaces 80±29 (range, 49-135) days or 84±18 (range, 52-112) days following Zilver PTX (20 stents in 10...

Drug-Coated Balloons for Long Superficial Femoral Artery Disease: Leaving Nothing Behind in the Real-World.

SEE PAGE 950 T he optimal endovascular strategy for treatment of superficial femoral artery (SFA) atherosclerotic disease is an evolving area of research and clinical practice. Initial randomized trials demonstrated a benefit of nitinol self-expanding stent placement in this region when compared to standalone balloon angioplasty (1). However, observational studies subsequently raised concerns r...

Sirolimus-eluting stents for the treatment of obstructive superficial femoral artery disease: six-month results.

BACKGROUND Stent implantation for obstructive femoropopliteal artery disease has been associated with poor long-term outcomes. This study evaluated the effectiveness of shape memory alloy recoverable technology (SMART) nitinol self-expanding stents coated with a polymer impregnated with sirolimus (rapamycin) versus uncoated SMART Stents in superficial femoral artery obstructions. METHODS AND ...

In-Stent Restenosis After Femoropopliteal Interventions With Drug-Eluting Stents: Same But Different?

SEE PAGE 828 T he superficial femoral artery (SFA), the longest artery in the human body, is subjected to nearly constant torsion, bending, and compression. It is perhaps natural to expect it to be the Achilles heel of endovascular interventions, plagued by higher rates of in-stent restenosis (ISR) compared to other arterial beds. Indeed, the 12-month rates of ISR after implantation of modern b...

Critical evaluation of stents in the peripheral arterial disease of the superficial femoral artery – focus on the paclitaxel eluting stent

The endovascular management of obstructive disease of the superficial femoral artery (SFA) is challenging due to unique anatomical and biomechanical forces. Obstructive lesions of the SFA make up the largest proportion of lesions leading to symptomatic peripheral arterial disease. Accordingly, endovascular treatment of SFA disease is becoming increasingly common and, in many cases, is the prefe...
